    Cubitus valgus

    Diagnosis

  1. Right sided
  2. cubitus valgus deformity of 20 deg
  3. with tardy ulnar nerve palsy
  4. due to nonunion of lateral condyle humerus fracture
  5. of 1 year duration
  6. in a 12 year old right handed male school-going boy
  7. presently disabled due to weakness of right hand & cosmetic deformity
  8. History

  9. Injury ( time, mechanism, open/closed, treatment taken,)
  10. h/o massage done
  11. Deformity ( when noticed, progression)
  12. Pain
  13. Siffness
  14. Tingling, numbness, weakness
  15. O/E

  16. Nonprogressive Cubitus valgus deformity of 20 deg,
  17. 3-bony point relationship disturbed with increased distance between olecranon & lateral epicondyle
  18. Thickened lateral supracondylar ridge
  19. Abnormal mobility of lateral condyle fragment/ pain on stressing lateral condyle
  20. E/O tardy ulnar nerve palsy in the form of
    • Atrophy of hypothenar eminence
    • Weakness of FCU, FDP,ADM, Interossei
    • Sensory d
    • Palpate ulnar nerve
    • Tinel's sign at-
    • Sensory deficit in ulnar nerve distribution

  21. There is 20 deg flexion deformity with further flexion possible upto100 deg with a bony block, the range is associated with pain & crepitus
  22. There is 40 deg supination & 50 deg pronation possible
  23. Arm length is reduced by 2 cm
  24. 2 cm arm wasting of arm 7 forearm
  25. e/o valgus instability
